Privacy breach notification form

Before you begin

The tool works best with up-to-date internet browsers (e.g. Chrome, Firefox and Edge). We recommend not using legacy browsers with it (e.g. Internet Explorer and legacy Edge).

This form is only for organisations to use to report a privacy breach to us.

  • If you want to notify us about a privacy breach of your own information, or on behalf of someone about a breach of their personal information, please make a privacy complaint. 
  • If you have received someone else's information or you want to alert us to a privacy breach by an organisation but you are not reporting it on their behalf, please contact us on 0800 803 909 or use our secure online enquiries form.

If you are reporting a privacy breach by an organisation, on behalf of the organisation, please continue.

Please take care when you tell us about your privacy breach that you're not also sharing people's personal information. We can help you without knowing that detail. If you're unsure then ask us before you share.

Please use only NotifyUs to report breaches so we can manage our workflow and get to your notification ASAP. You can attach documents in the NotifyUs tool. Do not email us your documents.

 

Completing a report  

  • You are not able to save a report to complete it later. You can see what information is required here. If there is information your organisation does not yet have, you can provide it later as an update to your report.
  • If you are unsure what answer value to choose for any question, we encourage you to err on the side of caution. 
  • We use end-to-end encryption to keep your submission secure. Nothing you report is stored on our website.
  • All fields are required unless indicated otherwise.

After you submit a report, you will get an automatically-generated email acknowledgment, which includes a submission code. If you don’t receive this email then it’s because there was either an error in the email address you entered, or the submission has failed. If a second attempt also fails, please email us at: notifyus@privacy.org.nz
